Informed Consent and Release of Liability
By signing this document, I acknowledge that I have voluntarily chosen to participate in a program of progressive physical exercise. In signing this document, I acknowledge being informed of the strenuous nature of the program and the potential for unusual, but possible, physiological results including, but not limited to, abnormal blood pressure, fainting, heart attack, or death.
My signature indicates that I expressly agree that all use and exercise shall be undertaken by me at my own risk and that Conor Maguire, Inc., dba Gymworx, shall not be liable to me for any claims, demands, injuries, damages, actions or causes of actions, whatsoever, to my person or property arising out of or connected with the use by me of the services and facilities of such corporation on the premises where the same are located and actions or causes of actions and from all acts of such corporation and employees.
By signing this document, I acknowledge that I am in good health and that I have no condition which would be worsened by participation in Gymworx. Further, I warrant that should I develop a health condition which could be aggravated by participation in this program, that I will immediately notify Gymworx in writing of such condition.
By signing this document, I assume all risks for my health and well being, and hold harmless any responsibility by an instructor, Conor Maguire, Inc., Gymworx, or any persons involved with this program and all included procedures.
I understand that questions about exercise procedures and recommendations are encouraged and welcomed.
The undersigned expressly acknowledges that this agreement is non-cancelable and non-refundable. The undersigned also understands that failure to regularly attend all Gymworx sessions does not obligate nor result in a partial or full refund from Conor Maguire, Inc., or Gymworx.
